Children's Mental Health Program:

A wide variety of services are available to assist children, youth and families coping with mild to moderate mental health issues. Individualized assessment, counselling, case management and follow-up services are available to French-speaking children who have been exposed both directly and indirectly to some form of trauma such as violence, abuse, intimidation, war or torture.

The Service Coordination Program provides assistance to children and youth who require specialized mental health services that are currently not available in French. Services in this program may include: accompaniments and interpretation services, assisting English-speaking mental health professionals in the development of case plans for French-speaking clients, carrying out client assessments, counselling/treatment and case management services as agreed upon and in collaboration with the English-speaking service provider.

Adult Counselling Services:

Counselling services for those facing issues such as depression, bereavement, relationship difficulties with children etc.

Provides counselling and transitional support services for individuals identifying as women and for women who have been or are currently victims of abuse and/or sexual abuse. Also offering services for men who have been victims of abuse and/or sexual abuse during their childhood. Practical assistance such as accompaniments to lawyers' appointments, referrals to shelters or other housing options and safety planning are available. Off site services may be available to abusive partners through the Partner Abuse Response (PAR) program.

The CSC also partners with a 24 hour French Language Crisis line for victims of woman abuse "Femaide" 1-877-336-2433 1-877-FEMAIDE.

Patients must be referred to the program at West Lincoln Memorial Hospital by a Physician. COVID-19: Hospice, programs and services continue to be offered. In-person programs are being offered virtually and there are safety protocols for the hospice residence. The administrative office is open weekdays from 10 am-4:30 pm with limited onsite staff. Visit website for details.

Assists people and families from the time of diagnosis with a terminal illness, through to end-of-life care and bereavement. Programs and services are free of cost and support people in their homes, in the community and in the hospice residence.

Care at Home: Working with primary care providers as one team, medical, emotional and social supports help people live well, for as long as possible in their own home, while supporting caregivers.

Care at Hospice: A 10-bedroom home, providing care, compassion and dignity for people at end of life and their families.

Care in the Community:

Wellness Programs: Programs such as Day Hospice focus on helping people and caregivers live well.

Bereavement Support: Children, teenagers and adults explore their feelings of grief and loss in a safe and supportive space. Participation is not dependent on their loved one having been a Hospice Niagara client.

Education and Consultation: In partnership with the Palliative Pain and Symptom Management Consultation Program, professional palliative care education and consultation is delivered to service providers across Niagara.

Phone line that anyone in the community can call to help connect at-risk older adults with programs and services. The Niagara Gatekeeper program works in collaboration with local partner agencies.

Referrals are made to the most appropriate partner agency. Upon consent from the older adult, the partner agency will provide education around local programs and assist in the coordination of referrals to a range of services and supports.

Services are intended to help individuals remain safe and independent at home for as long as possible. The referral line is confidential and callers may remain anonymous..

A senior showing the following signs may need assistance:

difficulty communicating, including confusion, memory loss, becoming withdrawn, hostile or angry
changes to personal appearance, including poor hygiene, grooming, wearing dirty or inappropriate clothing
deteriorating home conditions, including extreme neglect or disrepair, bad odoiurs, excessive clutter, neglected pets, lack of food
deteriorating health such as difficulty seeing, speaking or hearing, poor mobility or balance, recent falls, significant weigh loss, injuries or untreated wounds
decreased ability to handle money or pay bills
unusual transactions
inability to cope with the recent loss of a spouse or family member, friend or pet

This service provides a quality in-home respite service to individuals with Alzheimer disease or progressive age-related dementia. Trained respite companions provide meaningful activities tailored the client's needs and interests, in the client's home.

The program aims to provide:

Activities that bring enjoyment, purpose and meaning to the client's life
Socialization and interaction to maintain function and reduce risk of depression and loneliness
Companionship to increase client safety and provide reassurance
Respite for caregivers

Respite Companions may provide the individual with:

Help with meaningful activities such as walks, maintaining hobbies, socializing or other activities according to his/her ability, need and desire to participate
Prompts for medication may be provided according to program policy
Interaction that helps maintain functioning and enhances self-esteem which may decrease feelings of depression or loneliness
Preparation and serving of snacks, beverages and light meals

Respite Companions are not able to:

Provide personal care such as toileting, transferring, feeding or bathing
Drive clients in their vehicle, perform banking or other financial transactions
Clean the client's home, do laundry or shop unless it's a shared activity with the client

A program that helps older adults live active, healthy and independent lives. The South Niagara Health and Wellness Centre is a partnership of community and health care providers offering rehabilitation, wellness education and community resource connections to help older adults become healthier, safer and stronger.

The service provides opportunities for older adults to:

Access professional services such as Physiotherapy, Occupational Therapy and Speech Language Pathology
Build strength, balance and confidence through exercise
Socialize with others who are motivated to stay healthy, safe and strong

Specializes in enhancing the lives of older adults living with Dementia and behavioual changes.

The Behavioural Support Transitional Unit provides temporary accommodation in a home like setting for up to 17 clients. Assess the emotional, social, enviromental and physical needs of each client and then develop individualized treatment and care plans that prepare clients for their relocation to the most appropriate care setting.

The centre has a dedicated physician for the programme who works with the team to identify appropriate treatment and goals for each individual.

The purpose of the Centre is to provide care for variable short periods of time in the Regional Centre with the goal of returning the client/resident with appropriate supports to their local community or facility setting.

ACCESS AND CARE - COMPLEX CARE CENTRE: The Complex Care Centre can accommodate residents who are living in Niagara for up to 90 days (with a possible extension of an additional 90 days).
